Calyx Point 5.4

When I try to print the GFE the signature lines are cut off, I adjusted the print offset so they would print, which worked.  I am now not getting the initials lines on the bottom of page 4. I cannot adjust the print offset any farther without cutting off the top of the form.  Can anyone tell me what I am doing wrong?

I am not able to create a print preview that is right.  Do you think it's a problem with the printer?
