| I was told, by Calyx support, that an Electronic update will be issued to update this form. They could not tell when it would be available.

| Solution for updated HUD 92564 - CN:- Download form from HUD's Website & save to desktop.
- Convert document from pdf to Word.
- Import into Point.

| | That's great, but we don't have a way to convert PDF to Word without losing all of the formatting. We took a screen shot of the PDF and pasted it into Word as an image. ...works for now. Regards, |
