I was told, by Calyx support, that an Electronic update will be issued to update this form.  They could not tell when it would be available.

Solution for updated HUD 92564 - CN:
  1. Download form from HUD's Website & save to desktop.
  2. Convert document from pdf to Word.
  3. Import into Point.

That's great, but we don't have a way to convert PDF to Word without losing all of the formatting.

We took a screen shot of the PDF and pasted it into Word as an image. ...works for now.
