We have seen an increase in missing files in our PDS environment.  Over the past couple of weeks, there have been enough "I'm missing a file" requests to become alarming to me.

How and why would files go missing from PDS.  Assume first that they didn't get manually deleted.  How else would files which have been in the queue for months go missing?

When you say missing what do you mean exactly? Are they no longer showing up for the user? Did you do a folder refresh and they still not show up? Have you checked the users machine to see if perhaps there was a copy left behind there that could be re-uploaded?

We ran into some issues previously with a glitch that when you just clicked save then closed Calyx it wouldn't actually commit the file back into the PDS side of things and it would be "missing" until a reindex of PDS.
