Is there a way to modify a calyx form? Or should I just create a custom form. I want to fill out more of the 4506-T to include the dates and also checking a 2 of the boxes. Is this possible?

This was an older question, but now that some investors are requiring the 4506-T to be completed and signed at application (or at least with disclosures) there needs to be a screen to allow the fields on this form to be edited.

Please help.

You would need to create a custom form. There's currently no way to enter those extra fields in Point. I have added your request to the feature list. Thanks for your post.
