We had a auditor notice an error on the Informed Consumer Choice Disclosure Notice form provided by Calyx - Form ccdisc.frm dated 1/01 - The last statement reads as follows:

If you were NOT (Not is in Italics) charged a up-front premium, as for example on condominiums, you will pay the monthly premium for the life of the mortgage.

Condos for FHA now do pay the upfront, so this statement needs to be removed.

Can this form be fixed and posted to the support website?  Thanks.

Are there any plans to update the Informed Consumer Choice Disclosure Notice anytime soon.  The information needs to reflect the recent changes in the UFMIP.  It would certainly be nice if Calyx created a download.....hint, hint, hint!

Have you tried going to Utilities > Company Defaulfts > FHA Consumer Choice Disc? It lets you change the defaults of the UFMIP and more...

Hope that helps

That is not the issue.  This problem is with the wording of the actual document.  Because of Mortgagee Letter 2008-22 effective Oct 1 revised the moratorium of risk based premiums the form in Point is no longer correct.

See Mortgagee Letter 2008-22.
